No Show/ Cancelation Policy
This office has a strict no show/late and cancellation policy.
We at Headlight Mental Healthcare work very hard to respect your time and commitment by doing everything in our power to keep your appointments in a timely manner and avoid unnecessary rescheduling. It is our hope that you will do the same by following the guidelines outlined below:
- Provide 24 hours notice prior to scheduled appointment for a full cancellation refund. You agree to the appointment time and date when it is scheduled so technical issues with appointment reminders, texts, or emails does not excuse you from the personal responsibility to keep your appointment.
- If you are more than 10 minutes late, your appointment will need to be RESCHEDULED. We try to be understanding, but we have other patients that would be inconvenienced if you do not show up on time.
- After your first No Show/Late Cancellation, you will receive a letter reminding you of our policy via the patient portal. This is considered a warning that the policy will be put into effect at any subsequent missed/late appointments.
- If this becomes habitual you are subject to termination by the provider or clinic. A certified letter and final 30 day medication supply refill provided.
